QR codes?

I do. I use a company named AirVote. They set everything up for a fee. Check them out at air-vote.com

Customers can send a happy, so so or sad votes anonymously. They can also add photos, call back numbers and suggestions.

How much does it cost?

My main idea is I want them to scan that if their product doesn’t dispense. Can it do that?

My next idea would be if they request a product.

The yearly fee depends on how many people use them and the QR codes themselves were $300 for 100. They are very high quality decals. I think my first year was 350.00 plus the $300 for the decals. 
You get so many votes before they charge you.

I would prefer something I don’t have to pay a subscription for. 

I am thinking of trying out a QR code that redirects to a Google forms. Then I get an email every time someone fills it out.

Then google that.  I'm sure there are free apps out there that will create QR codes.  You might have to do some programming though.  Nothing is really free, you know.

All I want to do is put a QR sticker on my machine that when scanned it goes to a form they fill out saying they did not received their product, or they are requesting a certain product.  Etc. 

I don’t plan on using it for purchase or anything like that.

If you just have one machine, AirVote gives you a couple freebies to try out

I have had someone design contact stickers for me in the past with QR codes on them that were generated on free websites. They redirect to my contact page on my website. Used UPrint.com, was about $100 for 100 of their stickers.
