Here is a sample which I saw in Automatic Merchandiser. Hope it is helpful to you. It is a word doc so you can copy and paste it. Then made changes as you see fit. Vending Machine Installation Agreement THIS AGREEMENT executed on this the day of , 20 by and between (hereinafter "Business Representative"), and (hereinafter "Vendor"). NOW, THEREFORE, FOR AND IN CONSIDERATION of the mutual promises and agreements contained herein, ____________________gives vendor permission to install the following vending machines____________________________________________ at this location_________________________________________. You agree to provide the necessary electric and space for each machine. You also agree to provide our machines the same care and responsibility as you would you own equipment. You also acknowledge that these machines are owned solely by the vendor____________________________________. The Vendor agrees to service, stock and maintain the machines in a professional manner. The Vendor agrees that repairs will be handled on a priority bases. This agreement will be in place for a period of one year and will renew unless you contact us within 60 days of the renewal date to remove our equipment. Product pricing will remain the sole responsibility of the vendor. You will be provided with a product list from which you can select items for your equipment. You will appoint one person at your location to approve all product select changes.
